Abatacept (Orencia) Options

I am in an awful state!!! I have just seen my rheumy who says I need a drug change.

Having failed my second anti-tnf I am now about to start Abatacept (orencia) in a couple of weeks. Does anyone have any experience with this drug? And how long does it take before it kicks in? I have just read that Orencia could take 12 weeks to show any benefit, how I am going to survive this I don't know. I am not far off getting the wheelchair back out which I thought I wouldn't need again, after having my hips and knees replaced. My joints are just exploding with pain at the moment, I have oral steroids to help me out but I can't stay on them long term. The awful thing about these drugs is there is absolutely no guarantee they will work, it's just such a gamble. I am trying to stay positive but my mobility is deteriorating rapidly. I am taking so many painkillers.

I would be so grateful for any comments from anyone on this drug.

Hi Rebecca - I started on 31st Jan, having failed cimzia, enbrel and rituximab. No side effects yet but no difference either and I'm going to have to stop in 2 weeks while I have the foot op. Hope it works well for you xx Ailsa

so sorry to hear how badly you're struggling,

that's the trouble with each new drug, have to wait a good 12 weeks to see if they work,

i know others who no longer post have been on it, have no experience myself.

i know you're taking steroids, can you ask for a depo steroid injection to help tide you over? if they work, they can work very well.

Hello Rebecca

Sorry that you are going through the mill.
We are all different & everyone's ra is at different rates/various stages. My ra is erosion.....which isn't great. I'm fortunate at tube moment with my ra behaving with enbrel.

Anti tnf's normally take a few months & several blood tests to see the number crunching improving.

I certainly echo thoughts of depo steroid - a welcome short term fix of dampening down and so e normality :)
Remember, candles, relaxation (if you can) and some chocolate/cake lol

Don't forget so much research/trials go on for chronic conditions like ra, please try to have some pamper time.

I'm not in remission, too active to be settled....I try to chill a bit but hc@h cheese me off with late deliveries/no contact etc ;(

Is abatacept the infusion meds? Just being nosey lovely.

Thank you to you all for your helpful comments and support.

Just out of interest Ailsa, are you taking the abatacept by injection or infusion? I will be on injections. Did they give you any idea how long the drug takes to work? 12 weeks I have been told but reading the internet it could be six months.Scared Best of luck with the foot op, I hope it is successful and you can get back on the Abatacept quickly.

Thanks for your advice Suzanne, can I have a depo steroid injection if I am taking a course of steroids? I will have to find out. I think I will just need a permanent drip of steroids to get through the next few months.

Glad the Enbrel is helping you Jane, out of all the drugs I have been on in 14 years Enbrel was by far the best. I was on it 8 years and then it stopped working. Abatacept is an infusion but it has recently been developed as a weekly injection which I have gone for because I live an hour away from the hospital and I can't drive far. Luckily the injections are not coming from hc@h as I have had a nightmare with them too.ThumbDown

hmmm don't know the answer about having a depo injection while on steroids to be honest,

i've never had to take steroids, maybe worth asking though.

hope Healthcare improve, they delivered on time and answered the call straight away for me re my delivery due this Friday after a nightmare the previous time.

thought they had gone back to normal but after reading Jane's post not so sure now!

Hi Rebecca - I am on weekly injections as I couldn't arrange the monthly infusions around work. I'm also on a research project at the hospital looking t the success of injections compared with infusions. I was told to expect improvements after 3 months so I'm only half way there and will only have on more next week before I have to stop for a while.

Hope you ar as trouble free as I have been and it works wonders for you too xx Ailsa

Hi, I have been on abatacept for 18mnths or more now.
I have it by monthly infusions, my hospital has not gone onto inj yet, but I have been asked if I would be happy to use them.
I failed with Enbrel and RTX and had been really struggling with uncontrolled ra for 2-3 yrs.
Abatacept has worked brilliantly for me, and I felt the benefits of it fairly quickly.
I have just started on it again now, after a break, because I am 6 weeks post op after having a knee replacement.
My ra is flaring again now, but should be more controlled again soon.
Hope it works well, and quickly for you. I have no side effects from it at all.
Let us know how you get on with it.

Did you have a loading dose on the injections Ailsa when you first started? It sounds like you are coping well on them. Best of luck with your op, how long will you have to be off the Abatacept for?

Zena, that is really encouraging for me to read your post, how long did it take before you noticed an improvement? I am losing my mobility so quickly and hoping to start the treatment in a couple of weeks. Hope you have a successful recovery with your new knee. My new knees were fantastic until my drugs stopped working. Have you been off the Abatacept long?

Thanks Sophie for your thoughts and wishes. I am still waiting for the abatacept delivery. It's just such a nightmare isn't it when the drugs stop working. Have you had your other knee replaced yet? I can't believe how swollen my knee replacements are at the moment, doesn't feel like they are replaced nowSad Just hoping new drug will work,I am living off steroids at the moment.

I know what you mean about swollen knee replacements , I had a flare couple of weeks ago and couldn't even bend my left knee !
How long have you got to wait for the Abatacept ? Hope it's not to long now .
My right knee is booked in for 13 th May Scared , but I'm getting more pain in my right lower back / bum cheek , just rang rheumy nurse to ask for my right hip X-ray results (I had taken when last in clinic ) but she says she can't tell me them and to ring the consultants secretary to find out . I need to know why I'm getting pain there and if theres some thing that can be done to stop this pain before my knee replacement .

Hi Sophie,

One possibility is that the pain in your right hip is caused by you compensating for your right knee. If that's the case then maybe when your knee is done your hip will improve? (I'm trying to look on the bright side here! Smile )

Hi Sophie

Paul is right, I had awful pain in my lower back and hip replacements before my knees were replaced, it all subsided once my mobility improved. Your posture is probably all out of alignment, hopefully when your knee is replaced it will improve your overall pain.

It would be helpful to get the x-ray results for your hip though. If ever you need a hip replacement in the future Sophie having a hip done is a breeze compared to a new knee. I can feel your anxiety, knee replacements are so challenging but so worth it. Keep thinking of all the things you will be able to do once it is replaced. May is a good time for an op, you have got the summer to recover and enjoy a new pain free knee.

Hoping the delivery company will ring next few days with date of abatacept. I don't know how I will get through next 3months.

the nurse said that to , i just can't understand why I can be in really bad pain in my lower back and hip area (more than my knee) and have to get knee done first ! That's really good news that the pain should subside when my mobility improves Rebecca .

Been ringing consultants secretary but she is not answering phone , will try on Monday now for results of X-ray .

I agree May is a good time for op , seen consultant in January could of got it done earlier but my sister is due to have a baby in 2 weeks ,so my mum and other sister will have to help her ( she already has a 15 month baby boy ) and my sons is nine in April so didn't want to be in pain for his birthday .

Good to know hips are a lot easier than knee ops ThumpUp

hope you get the Abatacept soon and it works quickly for you Rebecca ,

The problem is mechanical. You are walking with a gait which means that one side is taking more pressure than the other. That impact is transmitted up your thigh bone into your hip. The unequal pressure puts strain on the joints and muscles in your hips and pelvis so you compensate even more.... and so it goes on.

Hi Rebecca,

Yu can have a depo injection if you are on oral steroids, they will just lower your oral dose after your injection.
They do make a big difference and last quite a few weeks if you're lucky.
Worth a try!

Thank you Gogs. I will need a depo injection as soon as I stop the cimzia and start abatacept. I don't suppose you know how often I could have a depo injection because what joints I have left, are going to be eaten alive in the 3 months waiting for abatacept to work.

hi Rebecca,

i think you can have depo injection with a 6 week gap from memory.

i've had about six , three worked well and three didn't. i came out in hive's with the last two not many but enough to put me off.
